For new prospective students. Not necessary for returning cadets.
The American Red Cross and Virginia Blood Services conduct annual blood drives at Fork Union Military Academy. Cadets who are at least 17 years of age and weigh at least 110 pounds are eligible to donate blood. These agencies continue to have an urgent need for blood products and are seeking potential donors.
All health forms should be completed prior to enrollment at FUMA and must be completed in English.
The above forms and information pertain only to Middle School cadets and their parents.
